Improving Hypertension Management for High-Risk Populations in UPMC’s Primary Care Network

Uncontrolled hypertension affects many people. Patients’ experience can inform an equitable approach to improving hypertension management at real-world primary care practices.

Study Description:

The study aims to improve blood pressure management in high-risk patients with uncontrolled hypertension (e.g., comorbid diabetes, minority race, etc) through the implementation of the American Heart Association's guideline-based care, regular blood pressure monitoring, and medication intensification. This is an implementation (i.e., quality improvement) rather than a research project. We seek patient partners to be named on the project proposal to inform implementation strategies and help the project team interpret study findings and understand the scalability of hypertension management. The proposal is due September 4th and the project is anticipated to start in Fall 2025.

What's involved?

During the proposal development phase (now - September 4, 2024): connect with the project team for 30 minutes to 1 hour to discuss the project and expectations for involvement when funded. This meeting will not be compensated but we will discuss compensation during the project. During the project (4 years starting Fall 2025): engage with the project team regularly (cadence can be discussed) to inform quality improvement efforts and patient engagement approaches. These activities will be compensated as part of the grant funding.

Adults with hypertension diagnosis & had experience with uncontrolled hypertension. We encourage minority racial groups (Black, Latinx, Asian) to inform this project.
